Paul Lieverse, anesthesist: ”Dit is mijn roeping: mijn werk goed doen”
Bij het thema ‘medische ethiek’ denkt men al gauw aan klassieke hete hangijzers zoals abortus of euthanasie. Wetenschappelijk tijdschrift Radix sprak met Paul Lieverse (68), gepensioneerd anesthesist en momenteel raadslid voor de ChristenUnie in de gemeente Lansingerland. Wat voor medisch-ethische kwesties spelen er op het vlak van pijnbestrijding? En wat voor betekenis heeft het christelijk geloof in de medische beroepspraktijk? Hoe is de keuze voor anesthesie ontstaan?
Ik twijfelde tussen wiskunde of geneeskunde. Het werd de studie geneeskunde in Utrecht. Daarna volgde de anesthesieopleiding in Nijmegen. In mijn studententijd raakte ik geïnteresseerd in allerlei vormen van spiritualiteit. Zo volgde ik workshops boeddhisme en yoga bij meditatiecentrum De Kosmos Amsterdam. In de mensa (studenten-eethuis) in Utrecht raakte ik in gesprek met een student van studentenorganisatie Navigators. Hij vroeg mij: je onderzoekt van alles, waaromgeef je de Bijbel niet een kans? In die periode had ik ook contact met een student van studentenvereniging Ichthus. Daar is voor mij gebeden, dat ik tot geloof zou komen. En zo is het gegaan.
Waarom het christelijk geloof mij raakte? Jezus was voor mij voorheen een goeroe, zoals zoveel andere goeroes. Tot ik Johannes 11 las waarin Jezus huilt om de dood van zijn vriend Lazarus. Er knapte iets bij mij: Jezus is niet ongenaakbaar en ver verheven boven het lijden van mensen. Voor Jezus bestaat het lijden wel.
U bent gespecialiseerd in pijnbestrijding. Hoe benadert u als arts pijnklachten van patiënten?
Ik leg graag nadruk op het onderscheid tussen acute pijn en chronische pijn. Acute pijn is doorgaans zichtbaar, bijvoorbeeld veroorzaakt door snijden of branden. De patiënt heeft een verwrongen gezicht of transpireert. Bij chronische pijn kan iemand murw zijn gebeukt door de constante pijnervaring en daarom vraagt deze persoon er minder aandacht voor. Dit is een valkuil: je kunt denken dat het wel meevalt. Daarnaast is de context van pijn belangrijk. Iemand kan veel meer pijn verdragen na het horen van goednieuws, bijvoorbeeld na de boodschap dat de operatie is geslaagd. Mijn specialisme is pijn bij kanker, veroorzaakt door de kanker zelf of door de behandeling van kanker. Het is mijn taak om pijn onder controle te krijgen, maar ook om hoop te geven. Ik vertel wat de pijnbehandeling inhoudt, maar ook, mocht het effect tegenvallen, welke mogelijkheden er verder zijn om de pijn te bestrijden. Met dit perspectief verdragen mensen ongemak beter.
Bij goede medische zorg hoort mijns inziens ook de aandacht voor het mentale welzijn: uitleg geven, vertrouwen winnen.
Anesthesie gaat ook over palliatieve zorg. Wat verstaat u onder goede palliatieve zorg?
Palliatieve zorg is zorg die erop gericht is om lijden in de laatste levensfase te verzachten. De tijd die nog rest zo goed mogelijk doorkomen waarbij het gesprek over het naderend sterven niet ontweken wordt. Want palliatieve zorg is breder dan pijnbehandeling, het gaat ook om psychische zorg, zoals het luisteren naar boosheid: “Als ik eerder was doorverwezen, lag ik hier niet” of “Als ik niet had gerookt, was mij dit niet overkomen”. Of naar angst: “Hoe moet het straks met mijn kinderen?”, “Ben ik nog belangrijk, ben ik niet tot last?” Deze niet-lichamelijke dimensies zijn onderdeel van de zorg door arts en verpleegkundige en eventueel ook door psycholoog of geestelijk verzorger.
Hoe ziet u de verhouding tussen palliatieve zorg en euthanasie?
Hoe meer mensen weet hebben van goede, palliatieve zorg in deskundige hospices, hoe minder interesse er is in euthanasie. Wanneer angst vermindert, verdampt de vraag om euthanasie. In hospices en verpleegtehuizen wordt bijvoorbeeld maar sporadisch euthanasie toegepast, het vindt vooral plaats in de thuissituatie. Mijn stelling is: een mensenleven is de moeite waard, tot en met de laatste dag. Maar ik wil bij mensen aan blijven sluiten, ook wanneer zij hier anders in staan. Ik heb nog nooit iemand in de steek gelaten. Overigens zijn er ook niet-christelijke artsen die zich verzetten tegen de populistische benadering van het debat rond voltooid leven: er wordt gesproken over euthanasie voor iemand die ‘echt niet meer wil’, bij extreem lijden ‘waar niets meer voor is’. Maar wat houdt dat in? Het is nogal contextgebonden, een doodswens blijkt veranderlijk te zijn. Ik vind dat we te bang zijn om deze discussie open te voeren, ook binnen de christelijke politiek.
Zijn er medisch-ethische thema’s onderbelicht?
Een thema dat mij raakt, is: moet alles wat kan? Dan heb ik het over ingrijpende behandelingen die worden uitgevoerd, waarbij ik de vraag stel: is het wel zo goed? Mijn moeite is dat patiënten de indruk wordt gegeven dat er maar één stappenplan is: “Zo gaan we het doen.” Natuurlijk, formeel wordt om toestemming gevraagd, maar er wordt onvoldoende uitleg gegeven. Zo wordt chemotherapie bij borstkanker gepresenteerd als een noodzakelijke behandeling. Dat is lang niet altijd zo: het beoogde effect op de overlevingskans is gering, terwijl de bijwerkingen zeer heftig kunnen zijn. Ik vind dit een groot ethisch thema dat te weinig wordt besproken. Het is de taak van de arts om goede voorlichting te geven over de verschillende mogelijkheden. Maar ik zou graag mensen mondiger willen maken, zonder in wantrouwen te vervallen.

Hoort pijn bij het leven?
Een betere vraag is: hoort lijden bij het leven? Met pijnbestrijding kun je juist slechter af zijn. Ik ben als pijnarts gefocust om maatwerk te leveren: soms is het goed om een zekere mate van pijn te accepteren om nadelen van de behandeling te voorkomen. Tegelijkertijd zie ik mensenkrampachtig vasthouden aan de acceptatie van pijn, die gunnen zichzelf geen verzachting ervan. Dan wordt mijn taak te laten zien hoe men zichzelf en zijn omgeving daarmee tekort doet. Ik noem pijn beheersbaar wanneer iemand ermee kan omgaan, minstens nog kan genieten van kleine dingen: muziek, een bezoekje.
Hoe ziet u uw verlangen om te getuigen van Christus in relatie tot uw patiënten?
Ik begin niet zomaar over mijn geloof in gesprek met patiënten. Ik vind het belangrijk om een gelijkwaardige relatie met hen te onderhouden. Overigens vragen patiënten soms uit zichzelf naar mijn achtergrond, of ze weten dat ik christen ben en vragen door. Maar dit is mijn roeping: mijn werk goed doen.
Heeft uw werk uw geloof veranderd?
Ik vind het lastig om aan te geven in welke mate mijn werk, mijn studie of andere levenservaringen mijn geloof veranderd hebben. Als grote fan van John Stott herken ik de noodzaak van evenwichtig christenzijn; Stott noemt Bijbels realisten ‘reeds-nu-nog-niet-christenen’. Graag denk en droom ik in een ‘reeds nu’ (het Koninkrijk is gekomen) maar vaak ervaar ik een ‘nog niet’ (het Koninkrijk komt). Mijn medische werk van de afgelopen jaren heeft me van beide werelden iets laten zien en in mijn huidige politieke werk besef ik dat dubbele ook. En waar het om medische ethiek gaat, ben ik steeds meer de noodzaak gaan ervaren om de hoge standaarden die Christus me voorhoudt te vertalen in de praktijk en de mogelijkheden te ontdekken om ze te vertolken zonder jargon.
Wat zou u tegen christelijke studenten geneeskunde willen zeggen?
Wees een goede arts, en als christen een verrijking voor je vakgebied en werkplek. Geneeskunde is wetenschap, de praktijk is geneeskunst. Goed luisteren is daarbij essentieel en is te leren. En verder: geneeskunst is zo’n ontzettend boeiend vak.
